Online appointment scheduling is a service provided for CURRENT patients of Memorial Clinical Associates (MCA) ONLY. This service will allow you to request an appointment for a NON-URGENT matter by filling out a form, then you will receive a confirmation email in repsonse.

IF YOU ARE A NEW PATIENT TO OUR PRACTICE OR IF YOU NEED TO BE SEEN WITHIN 1-2 DAYS, PLEASE DO NOT USE THIS SERVICE, instead call (713)407-3000 and schedule an appointment with appointment line.

After you fill out the information, our office will respond by replying to your email address. Please confirm the appointment by replying to the suggested date and time.

Please do NOT reply with any questions in regards to medical care or for refill requests. This service is intended for scheduling appointments ONLY. Cancellation are NOT accepted by email, please call one of the following numbers to reschedule or cancel an appointment:
